Address Change

A health practitioner must promptly give the Registrar of the Optometrists and Dispensing Opticians Board written notice of any change in the practitioner's postal address, residential address, and if applicable work address.

To advise the Board of an address change either:

  • Fax, email or write to the Board informing it of your new address (residential, mail and/or work address), and include your registration number and date of birth.
  • Or change your address details online at our Change of Address page.

NB: The authority must from time to time publish the Register. The Register may include address information about a practitioner who has not objected to the authority in writing to the inclusion of that information, or who has withdrawn a previous objection to that inclusion.

Name Change

Within 1 month after a health practitioner changes his or her name, the practitioner must give the Registrar written notice of the practitioner's new name, along with verification of the changes e.g. marriage certificate or a deed poll certificate.
